Life stressors and self-concept Dr. Colleen McDonough and Rosemarie Griffith Neumann University 1 Neumann Drive Aston, PA 19014 Consent Form Invitation to Participate: I understand that I am being invited to participate in a study involving life stressors and the self concept. Basis of subject selection: Anyone over the age of 18 may participate . Purpose: This study is being conducted to gain a better understanding about how life experiences contribute to one’s feelings. Procedure: To accomplish the goals of the study the investigator will ask you to complete a survey online. The survey will ask you questions pertaining to your life experiences and your feelings of yourself. Why My Personal Health Information Is Being Used and By Whom: My personal health information will neither be collected nor used as a part of this study at any time Only the researcher will be permitted to disclose the information for the purposes of furthering science. The only people other than the investigators who may see your personal health data provided for this study are who may help the investigators to analyze the data, or prepare the study for publication or presentation. Once information is disclosed to others outside the institution the information may no longer be covered by the federal privacy protection regulations. In any disclosure outside Neumann University, your unique code number will be used rather than any name or other direct personal identifier. Your authorization in our databases, however, we will not use it for the other purposes, unless we specifically as you first. Will you be able to access your records: I understand that I will not be able access my records because the survey is anonymous. Potential Risks and Discomforts: I the investigator will try to make the study as comfortable as possible. The only risk identified in this study is the minimal discomfort from responding to personal questions. However, you are free to omit any answers or withdraw at any time without penalty. Can I change my mind about participating? I am free to withdraw before or during the study at any time without penalty Potential Benefits: Though there are no foreseeable benefits that will directly affect the subjects. This study will benefit both the psychology field and society as a whole by helping them understand factors underlying thoughts about life events and self-worth. Inducements: There are no inducements for participating Financial Obligations: All financial costs will be paid by the researcher Alternatives: As this is not a therapeutic study there are no alternatives Confidentiality and Privacy: I understand that since the survey is anonymous there is no possible way to link the results back to me. At no time will researchers release data to anyone other than the individuals working on the study. Only the investigators will have access to the data and the data will be kept under lock and key at all times. Non participation or Withdraw: I understand that I am able to consent or decline to participate in this study. If I agree to participate in the study I am free to withdraw from the study at any time. I understand that if I decline to participate or withdraw from participating there will not be any prejudice in future interactions between the persons involved and the investigator or the sponsoring institutions Complications or Injuries: There is little to no risk of complications or injuries present in this study. However, I am aware that compensation and medical treatments will not be provided by the investigator. Subjects’ Questions and Rights: I (the subject) have had an opportunity to ask questions regarding the study and/ or my rights as a subject of research.
