Festival of Neighbourhoods Registration Form
To register for the Festival of Neighbourhoods, complete the following form. Please provide as much detail as you can about your activity - others can get inspired when we publish the information on the FON Map and we can make sure to nominate you for all the awards you deserve!

If you have questions or have any additional information to share, flyers, photos or other media, please, send these to entries@festivalofneighbourhoods.ca or call 519-579-3800.

The information you provide in this form will be used only by Festival of Neighbourhood staff. Limited information may be used for Festival promotion and the FON Map. No information will be made public in any way that identifies individuals without their previous consent. Personal information such as name and address of contacts will be used solely for the purposes of direct communication and will not be released to any other party.

Eligible activities are those that:
• Tell us which streets or street boundaries make up the neighbourhood (the area, the building, the streets or a cluster of houses that were invited to the neighbourhood activity)
• Are open, advertised and welcoming to everyone in that neighbourhood, regardless of age, gender, ethnocultural background, ability, marital status, etc.
• Bring neighbours together face-to-face so they can get to know each other better

All registrations for activities held from October 1, 2016 up to September 30, 2017 must be received by midnight October 5, 2017!

Finale Recognition
Each year, at the Festival of Neighbourhoods Finale, we recognize neighbourhoods for elements important to build a strong and healthy community. Please answer the questions below so we can determine how your activity demonstrates these efforts.
Did you take the Inclusion Challenge? What ideas were explored or steps taken to make the gathering welcoming and accessible to all neighbours, regardless of their age, abilities, religion or ethnocultural background?
Please tell us a bit more about what you did to be more inclusive, and what difference do you think it made?
How were the arts included in the activity you are registering?
Perhaps you showed a local artist's work, included live performance, film, crafts, and artistic activities for different ages, etc.
