Holly Ridge Elementary PBIS Self-Assessment Survey
Each year, staff members at PBIS schools can anonymously fill out a survey to give their PBIS team an idea of how implementation of universal systems is progressing.  The survey is usually several questions but out of respect for your time, school and PBIS leadership has extracted 9 questions.  Please answer the following questions to help us understand how we can best work to serve you and our students.  You must answer if the items are in place, partially in place, or not in place at all.
