Grade 8 Literature & Language Arts Final Self & Course Evaluation, CISS 2011-2012, June 2012
Please carefully complete the following self-evaluation (part one).  Take a few minutes to reflect on your work in the literature & language arts courses during the LAST/4th quarter.  How would you evaluate your performance?  Please be sure to consider the entire fourth quarter (only April through early June).  Your responses will account for a portion of your fourth quarter grade in literature & langauge arts.  Your sincerity and honesty are integral in assuring that grades are kept fair and accurate.  Please complete the survey for YOURSELF and ONLY once BEFORE Friday, June 8th at 3:00PM CST.  Students who fail to complete an evaluation will not receive a mark (0).  Once you submit your answers, you will be unable to change them.   Email vicklauren@gmail.com should you have questions.

The second section of questions consists of a course evaluation has NO effect on your grade.  However, your answers can help make the course better (content, teaching style, evaluation style, etc.) for future students.  Your input and open, honest, constructive criticism are highly valued.   Be as direct and detailed as possible.  Include both positive and negative aspects with recommendations for improvement.  Thank you!
