Dear Applicant,

Congratulations on your interview at UCI SOM! In order to help us match you with a host for the evening before your interview, please fill out the following form at least a week (if at all possible) before your interview.

When making travel arrangements, please keep in mind that there is usually a Welcome Dinner for applicants at 7pm the night before the interview. This event is always fun and provides a great way to interact with fellow applicants and some current medical students.

We will contact you with a confirmation of your interview date and again as soon as we coordinate a host for you. Please note, however, due to the high volume of requests and the nature of students' schedules, we will not likely assign you a host until 1-2 weeks before your scheduled interview date. We appreciate your patience and understanding in the meantime. If you have any other questions or concerns, please let us know. <b>Additionally, if you happen to change or cancel your interview date, please notify us as soon as possible, as we are not notified by the Admissions office of your change in plans.</b>
