    I, being 18 years of age or older, do for myself do hereby release, forever discharge and agree to hold harmless Gracepoint Church’s Axis Singles Event,  and the directors thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damages and expenses, of any nature whatsoever which may be incurred by the undersigned participant that occur while said is participating in the above described activity.
    Furthermore, I hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein.
   Furthermore, I understand that no personal information will be given out unless mutual consent is given and therefore, aXis is not liable for the release of information.
   Furthermore, I have read the Axis statement of faith (see reverse side) and agree to it.

Statement of Faith
STATEMENT OF FAITH

As an Axis participant,

I believe the Bible is the written word of God, inspired by the Holy Spirit and without error in the original manuscripts. The Bible is the revelation of God’s truth and is infallible and authoritative in all matters of faith and practice.
[I believe that mariage is ordained by God for the
lifelong covenant between one man and one woman.]

I believe in the Holy Trinity. There is one God, who exists eternally in three persons: the Father, the Son, and the Holy Spirit.

I believe that all are sinners and totally unable to save themselves from God’s displeasure, except by His mercy.
[I believe that in my fallenness, I have a desire to fill my life
with things that don’t satisfy (including the opposite sex)
and that I need the grace and mercy of Jesus in my life.]

I believe that salvation is by God alone as He sovereignly chooses those He will save. We believe His choice is based on His grace, not on any human individual merit, or foreseen faith.

I believe that Jesus Christ is the eternal Son of God, who through His perfect life and sacrificial death atoned for the sins of all who will trust in Him, alone, for salvation.

I believe that God is gracious and faithful to His people not simply as individuals but as families in successive generations according to His Covenant promises.

I believe that the Holy Spirit indwells God’s people and gives them the strength and wisdom to trust Christ and follow Him.

that when I sin, my response in faith is to repent.]

I believe that Jesus will return, bodily and visibly, to judge all mankind and to receive His people to Himself.

I believe that all aspects of my life are to be lived to the glory of God under the Lordship of Jesus Christ.
