PDMAP Follow-Up Discussion Registration

Please provide all of the requested information below. We will confirm whether we have the required number of participants to conduct the discussion 1-2 days before the scheduled discussion. If there are at least three participants registered this email will include the agenda and the discussion questions by email. We use Adobe Connect Pro for the discussion . We do not place calls to participants. Participants will receive instructions for connecting to the discussion in the email that has the agenda and discussion questions. The two options for connecting with the discussion are going to a website or calling a toll free number. If you are connecting through the website, you will need to enter a phone number at which the system can call you that is a direct line. Telephone numbers with extensions or that are answered by a receptionist and have to be transferred CANNOT be used. If you will be participating with a group of staff members from your program, you will need a phone that has a speaker phone.
