Please fill out this form if your game was canceled and you need to reschedule. You must confirm with the opposing coach as to what days and dates will work before filling out this form. For all 6v6, 8v8 and 11v11 games, please check the "Home Field Game Schedules" on the website under "Fields and Schedules" for availability.

If you answered "YES" and it was due to weather, we will issue a check and mail it to the coach. This request automatically goes to the League Coordinator and once she schedules the request, it will be noted on the spreadsheet (far left column) - this will serve as your confirmation so please check back on the "reschedule" page of the website.
