2014 Radix Farm CSA Membership Application
WE ARE NO LONGER ACCEPTING CSA MEMBERSHIPS FOR THE 2014 CSA SEASON.  PLEASE EMAIL radix farm@gmail.com AND ASK TO BE PUT ON THE NOTIFICATION LIST FOR 2015.  YOU'LL THEN HEAR FROM US IN MARCH 2015 ABOUT NEXT SEASON'S CSA.  THANKS!!!!!


To apply for membership in Radix Farm's CSA, please read about the various CSA share options (found at http://radixfarm.wordpress.com/csa/) and then fill out this CSA Membership Application.

After we receive your form, we will email to let you know if you've secured your preferred pick up location and type of CSA share.  We will then email you the 2014 CSA Member Information Packet and Member Agreement.  You'll have seven days to mail your payment and signed copy of the Member Agreement to the Farm, which will finalize your membership.  

Shares are sold on a first come, first served basis.  We will keep a waiting list throughout the season.  If  additional shares become available, we'll contact folks on the waiting list.
