BetterGrads Program - Mentee Agreement Form
Thank you for your interest in participating as a mentee in the BetterGrads Program at your high school. Our goal is to connect you and your fellow classmates to peer mentors (college students and young professionals who attended your school or live/work in your area) who can support your college and career aspirations and personal advancement. Mentors are able to offer a real-world perspective on financial aid, admissions, transitions, academics, social life, jobs, and life beyond college.

Step 1. Fill-out and "digitally" sign this Mentee Agreement Form. This should take ~ 5-10 minutes. This form is online at http://bettergrads.org/program/be-a-mentee/.

Step 2. Ask your parent / legal guardian to fill-out and submit (either online or in hard-copy) the Parent / Legal Guardian Consent Form. This form is downloadable online at http://bettergrads.org/files/2010/11/Guardian-Consent-Form.pdf.

Step 3. We will connect you to a mentor, give you a monthly curriculum to cover with your mentor, and add you to the mentoring group at your high school.

I agree to regular communication with my mentor. I understand that this means a) at least one e-mail exchange per week, b) at least one phone call or video chat per month; c) at least one site visit per year (preferred, but not required); d) be available for monthly calls with other program participants; e) be available for regular group meetings with other mentees; f) active participation in this program for at least 1 year from the date I am assigned a mentor.

I fully understand that initiating or engaging in any inappropriate interactions with my mentor or other program participants will result in my expulsion from the BetterGrads Program, and possible further disciplinary action.
