I acknowledge and understand that my child will be engaging in an activity that involves the risk of injury. I hereby release and hold harmless the 2013 Catawba Classic Lacrosse tournament, directors, owners, sponsors, volunteers and facility sites from any liability involved with participating in the 2013 Catawba Classic Lacrosse Tournament. I knowingly assume all risks associated with participation, even if arising from the negligence of the participants or others, and assume FULL responsibility for my participation.

I certify that I, or my minor child, am in good physical condition and can participate in this event.

Further, I authorize the tournament director to request medical treatment as necessary to insure my child’s well being.

I agree to all terms in the US Lacrosse Code of Conduct, whose terms and provisions are incorporated herein by this reference, especially with regard to my responsibilities
as a player.

I understand that at this event or related activities I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by Fort Mill Lacrosse, Ltd., the event holders, producers, sponsors, organizers, agents and/or assigns.

This Participant Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I hereby certify that I have read this document and I understand its contents.
