7th Annual Dodgeball International Tournament of Champions Date: Sunday, March 3rd , 2013 Time: 2:30PM - 6:00PM Place: Hamline University - Lloyd Walker Fieldhouse Teams: 10-12 Players per team Cost: $50 Registration fee per team. $5 Registration fee per Free Agent All proceeds from this event will go towards the fund-raising efforts of Hamline's Catalyst Service Program. For more information about Catalyst visit: http://www.hamline.edu/catalyst/ INSTRUCTIONS: 1) Please complete all required fields and submit your teams registration form online by Saturday, March 2nd. 2) Team Leaders are responsible for ensuring all team members have read and agree to the "Assumptions of Risk and Waiver of Liability." All participants will be asked to sign the waiver form at check-in the night of the tournament. 3) Cash or Check Payments will be accepted at the time of the event. Make Checks Payable to: Hamline University. FREE AGENTS If you do not have a team, you may fill in the required fields in the "Team Information" section and you will be added to a pool of free agents, that can be placed on a team the day of the tournament. In the Team Name field below, type in "Free Agent." Please bring $5 with you for the Dodgeball Registration fee.
