Thank you for agreeing to be involved in the Hepatitis SA (HepSA) Northern Needs Assessment.  The responses in this survey will assist HepSA in assessing which services are most needed for people with hepatitis C in the northern suburbs and how HepSA can help in responding to these gaps.  Any responses you give in this survey will remain strictly confidential and no information will be identifiable.  You may answer as many or as few questions as you like.  Your answers and your decision to be involved will in no way influence your ability to access health care services at any health facility.  Services in this study refer to all clinical, allied health and support services for hep C.
