Arts Festival 2013 Submission Form
INSTRUCTIONS: Please read BEFORE you hit "submit."

* BEFORE SUBMITTING, visit http://www.bc.edu/content/bc/offices/artscouncil/festival/artsubmit.html.
*You must complete ALL fields on the form below.
* You must print and attach the filled in from BEFORE you hit "submit". Attach the printed form to the work that you leave in Devlin 415. Submissions without the proper information attached will not be considered.
* You must complete one form for each piece that you submit.
* The Arts Festival and the Fine Arts Department are NOT responsible for any work not picked up from the Stokes Tent by the artist or their representative by 8 pm on Saturday, April 27th.
* If your work is accepted, you are expected to attend the Gallery Opening on Thursday, April 25th, at 4:30 pm, and may be asked to give a 5-10 minute gallery talk.

Submit by April 19, 2013.
