PA Lunch Order Form - (9/17 - 11/15)

Lunch orders AND checks are due by Thursday 9/13!!

Thank you for choosing to participate in the PA's Lunch Program. If you're new to ordering, follow the directions below.

How to Order Lunch:
1. Fill in all the needed information and double check to make sure it is correct
2. Click "Submit". If you do not click "Submit", the order WILL NOT go through.
(All orders are electronic, so there is no need to print this form or the confirmation page)
3. Hand deliver your check payable to IDS PA to either Mrs. Montgomery or Mrs. McLaughlin.

REMEMBER TO TURN IN YOUR PAYMENT BY THE DUE DATE, OR EARLIER.
WE NEED TIME TO SPOT MISTAKES OR MISCALCULATIONS IN PAYMENT,
SO NO CHILD IS LEFT WONDERING.."WHERE'S MY LUNCH??!!"

Cupcake of the Month Club: A gourmet cupcake from My Dream Cupcakes is delivered at lunchtime to your child once a month on Cupcake Day. Cupcake Days are 10/15,11/15, 12/12, 1/15, 2/15, 3/12, 4/10, 5/15, 6/5. Pay $45 now for 9 months of a special treat and don't worry about it again! *

Please note: 1. This is one of the PA's fundraisers for the year, and it earns a 20%-35% return, depending on the cupcakes selected. Last year, more than $1000 was raised for the PA! 2. If you don't sign up now, there will still be an opportunity each month to buy that month's cupcake at a slightly higher price. 3. Accommodation can be made for an allergy cupcake if your child needs gluten free or even dairy and egg free. Please contact Jennifer Mahr at tjmahr@comcast.net to discuss your child's needs.
