Declaration

本人謹此聲明：
1. 本人給予生命前線有限公司（以下簡稱：生命前線）的資料均屬正確，於緊急事故發生時同意生命前線使用本人的個人資料作統籌。
2. 本人同意遵守生命前線導師的一切守則，尤其是於航行時及進行水上活動時遵守有關必須穿著救生衣的安全守則。倘若本人不遵守安全守則，本人同意生命前線終止本人或整個團體參與餘下活動而不需作出任何賠償。
3. 本人明白並願意承擔在此活動期間所有自身的意外風險及責任。若因本人的疏忽、或不遵守安全守則而引至本人發生意外、或財物損失，本人同意放棄追究生命前線、與及其職員、義工及其他參加者的任何法律及民事責任。
4. 船上所有物品及野外露營器材請小心使用，如有損壞或遺失，須照價賠償。
5. 本人知悉生命前線或會在活動進行期間進行拍攝或錄影，參加者因此有機會而被拍攝（除非事先提出），拍攝內容有機會作宣傳用途。
6. 本人參與是次旅程的身份是協助操作帆船的船員；所付的費用是活動程序費用，而非租船費用；並以私人借用的形式使用生命前線所提供的帆船。
7. 本人同意上述的一切條款，以及聲明本人身體狀況良好，並無任何傷患或不適影響本人進行戶外活動。

I hereby declare that:
1. All the information provided by me is correct, and I endorse the use of my personal particulars in case of any emergency during the program.
2. I agree to follow all the rules and regulations set by Life Front-line (the Organizer). Particularly, I will follow the rule to put on the life jacket during sailing and water activities, or at any time instructed by the staff members. If I do not follow the rules and regulations, I am aware that the staff member could terminate my participation in the program at any time. On this regard, the Organizer will have no responsibility to compensate for my incomplete program participation.
3. I understand and am willing to undertake any personal risks and responsibilities during the program. In any case of my own negligence or disobedience to the rules and regulations, I agree not to pursue any legal or civil liability to the Organizer, including the staff members, volunteers and any other participants in case of accident or loss of property.
4. I will handle all equipment and outdoor gear on the boat with care. I will compensate if I damage any of the items or equipment of the Organizer.
5. I am aware that the Organizer will take photographs and videos during our program. Unless stated otherwise, I now give consent to the Organizer for my image being recorded for possible promotional purposes.
6. I take part in this program to assist in the operation of the boat and sailing trip. I am clear that the fees paid are program fees instead of boat rental costs; I am aware that the boat provided by the Organizer is used as a private vessel.
7. I hereby sign below to agree on all the above mentioned, and that I am in good health condition without any injuries or illness which might deem inappropriate for my participation to the sailing program.
