University of Portland Service Plunge 2013 Registration Form

Welcome to the 2013 University of Portland Service Plunge Registration Form. You are about to officially register to spend your first days at UP participating in an incredible 3 day immersion serving the local Portland community, meeting your fellow first year students, asking key questions, and learning about how to get involved and make an impact during your time at UP. Volunteer work may include nature trail maintenance, food box assembly, community gardening, school clean-ups, painting, and a wide variety of behind the scenes support work that helps local agencies serve their clients. The Service Plunge is a time to have fun, meet new people, work hard, learn more about the Moreau Center, UP, and jump into service in your new community. We are excited to meet you and excited to introduce you to your new community, and your new neighbors, through service. TIME FRAME: Service Plunge runs from Tuesday August 20th- Thursday August 22nd. Check-in for the Service Plunge starts at 10am on campus. Students will return to campus by 10am on August 22nd. Please note that orientation for first year students officially begins at 6pm August 22nd. FORM TIPS: Please complete the form in its entirety and submit the form once complete. You will not be able to save this form so please be mindful when you are registering. We will only accept 1 registration form per student and a new wait-list form will be launched once the plunge has met its maximum capacity of 130 students. Registration will run from 10amPST Friday June 14th to 5pmPST June 28th. After registering through this form you will see a pop-up confirmation window - print this for your records if you can, you will receive an official confirmation email from the plunge coordinators shortly after you register. *Please note that it may take 1-2 days for you to receive this email, especially if you register over the course of a weekend. Please wait at least 2 days after submitting your form before calling the Moreau Center to confirm your registration status. Please Note:Due to program overlapping with the 2013 Service Plunge dates—Honor Students, ROTC, and those participating in the Ohana program cannot participate in the Service Plunge. Submitting This Form

What happens after I register? You should receive an e-mail confirmation from service plunge coordinators in 1-2 days. Because we only have space for 130 participants, we do keep a waiting list. If you are on the waiting list and a spot opens up for you, we will contact you by e-mail to confirm that you can attend. In early August confirmed participants will receive an email that will contain a link to a digital "Welcome Brochure" with details about the plunge, including what to pack, when to arrive, airport/train/bus pick-ups, and more. Additional information regarding these items and more can be found in the Service Plunge F&Q located at up.edu/moreaucenter. In addition—keep an eye out for e-mails from the Service Plunge Coordinators with other updates. Please note that the cost to participate in the Service Plunge is $50.00 -to be paid at check-in on August 20th CASH or CHECK ONLY.
