Hawai'i Pacific University (HPU) Multimedia and Cinematic Arts students please fill out this form for each project you have primary creative responsibility for. This is archival material, so please be as accurate and complete as possible. This form is to certify that the below mentioned student(s) is enrolled at the HPU Department of Communication in the listed course alpha-numeric and is engaged in the production of an authorized Digital Image / Design / Film / Video / Still Photography / CGI / Animation / Podcast / or other Multimedia project.

Rights, Title and Interest

This project is intended as an academic work and is not intended for commercial distribution or sale, Hawai'i Pacific University has waived all right, title and interest in and to this project, including but not limited to, underlying narrative rights for original stories. The student(s) named herein retain all right, title and interest in the project. Hawai'i Pacific University will be furnished an archival quality digital copy of all project-related work(s) through responsible faculty and/or staff; collateral digital documentation of the production process that is completed over the course of the class, and any significant changes that may come after the project is completed in class. Students also agree to list Hawai'i Pacific University and HPU Media and Cinematic Arts in project credits, unless informed in writing that this creditation is not required by the University. Hawai'i Pacific University retains the right to use, edit and reuse said project and/or its parts for non-commercial educational purposes, promotional or advertisement reasons in any and all media, now known or later devised. I understand that there will be no financial or other remuneration for use of project-related works. I waive any right I may have to inspect or approve any use of the project-related works and hereby release the University and its agents, employees, successors and assigns from any liability resulting from or connected with such use.
