Submission of this form constitutes an agreement between you & Frontwards Farm for 30 weeks of deliveries between March and December at the rates listed below. Please be sure you have read the detailed CSA info at:

I will be notified when you submit the form and will then send payment details and our mailing address to the email you provide.
If you sign up after Feb. 15, pay the first installment with the deposit.
Please send payments according to our arrangement. Our planning depends on it!
Farm pickups can be arranged for Friday afternoon.
Carbondale deliveries will be made on Saturdays at a time and place to be determined.
Maplewood deliveries will be made on Wednesdays (4-7) at the Schlafly Bottleworks.
If you choose to arrange regular add-on options (2nd to last question below) please estimate the frequency. We ask that you arrange and pay for these regular additions 15 days prior to the month in which you wish to receive them, and to commit to the entire month regardless of frequency. This will allow you to customize your share while allowing us to plan to better meet your needs. If you think you will want these items on a random basis, let us know now that that may be the case and still try to notify us at least 15 days in advance if possible.
