Player Tryout Signup - 2012 Extra Program SMAYSO
Thank you for your interest in participating in SM AYSO's EXTRA program for the Fall 2012 season. Please complete the form below for each child that wishes to participate in the tryouts. Additional information on the EXTRA program is be posted on our website under the Extra 2012 tab.

All tryouts will be held at John Adams Middle School (JAMS) according to the following schedule:

Boys U10-U14:
Sunday, April 29, 3pm – 5pm
Wednesday, U10 (5:00-6:45); U12 (6:30-8:00); U14 (7:30-9:00)

Girls U10-U14:
Sunday, May 6, 3pm – 5pm
Wednesday, U10 (5:00-6:45); U12 (6:30-8:00); U14 (7:30-9:00)

PLAYERS ARE ENCOURAGED TO PARTICIPATE IN AS MANY OF THE TRYOUTS FOR THEIR AGE GROUP AS POSSIBLE.


Age Guide for 2012-2013 Registration Year:
Division U-10  Age:  9               Born Between 8/1/02 and 7/31/04
Division U-12  Ages: 10 & 11    Born Between 8/1/00 and 7/31/02
Division U-14  Ages: 12 & 13    Born Between 8/1/98 and 7/31/00


PLEASE ARRIVE 20 MINUTES PRIOR TO START FOR CHECK-IN AND WARM UP.

There is no fee to participate in the tryouts if you were registered in AYSO during the Fall 2011 or Spring 2012 seasons.

For all others, you will need to complete the following:
  (a) Complete a medical release form at the first tryout (your child will not be able to participate without this form being completed and signed).
  (b) Register for the Fall 2012 season by going to the Registration tab on our website or by following the directions at the following link - http://www.ayso20.org/content/fall-player-registration. (If the link does not work, then copy ammmnd paste in your browser)


What to bring to tryouts:
1. Shin guards (required)
2. Soccer cleats (recommended)
3. Water and sunscreen

A reminder - JAMS IS A WATER ONLY FACILITY. Please do not bring other drinks or food onto the field.

I understand and acknowledge that Extra is a competitive soccer program and that playing in the Extra program requires a significant commitment of time and effort. Players must attend two practices per week from August to January (and through June if my child continues with the Spring season), attend games and support and cooperate with teammates and coaches. Parents must be willing to support their player's commitment to the program, to pay the Extra team and region fees and to accept a volunteer responsibility within the AYSO Program.
