Dlost Arts - Hi Res & Multimedia Commission Application
Greetings.
By filling in this form, you are applying for a commission offered by the artist, Dlost. Filling out this form does not guarantee you a slot.  The slot will only be reserved on payment in full (or appropriate deposit,  if applicable).  After you press “Submit”, the artist will respond as soon as they are able via email with order confirmation, questions, comments,  as well as invoice or other payment instructions if needed. All responses to this form will come from a "dlostarts.com" email address.

Mature and adult rated commissions will not be accepted from individuals under the age of 18.

Invoiced/Posted prices are not negotiable.  Questions about other options are, however, accepted.

False, improper, or otherwise missing information may result in your application being rejected.

Please be mindful that the artist reserves the right to refuse a commission on the basis of content.
