S-forms must be submitted by Monday at 5 P.M. prior to an event. Your organization will be notified if your S-form has been approved.
Final Guest List must be submitted by 5:00 P.M. on Thursday with the S-form via e-mail or hard copy.
Wristbands must be picked up from the Office of Student Life by Friday at 5 P.M. Failure to do so will result in the cancellation of your social(s) for the weekend. Any unused wristbands, Party Patrol t-shirts, and highlighted guest list (hard copy) must be returned no later than 5 P.M. the following Monday to Student Life.
You must put first and last name in all spots requiring a name. You may not put TBA.
