If you are a new or returning CSF member, you must submit a copy of your Fall 2012 report card, paper application and $5 BEFORE filling out this online application. Please make sure that you fill the application as ACCURATELY as possible.

Contact Information: The following information is used to contact you whenever announcements or tutor assignments are distributed. Please supply information that will help us contact you quickly and efficiently. *Inadequate means of contacting members will cause trouble for both the society and other students at Lowell, so please check emails frequently.
