University of Arizona Cheer Fest 2012 Group Registration Form

The Cheer Fest performance will be showcased at half-time of the September 29th football game against Oregon State University. Clinics will take place on September 23rd during the following times: 9am-12pm for Kindergarten through Fifth Grade participants and 2-5pm for Sixth Grade through Twelfth Grade participants. There is a fee of $50 per participant which includes a t-shirt, participant game ticket, DVD of the half-time performance, and the cost of the clinic. Fees sent through the mail should be postmarked no later than September 10th and sent along with a signed waiver to: Attn: Cheer Fest 2012 McKale Memorial Center PO BOX 210096 1 National Championship Dr #N302 Tucson, AZ 85721 Credit card payments will also be accepted (Please indicate below). Registration will be accepted through September 18th. Additional tickets are available in the same seating area as participants and can be purchased up until the game. It is recommended, however, that these seats be reserved now so you can ensure seating near your participant.

Participant Fee $50

Participant fee includes event t-shirt, clinic, Arizona vs. Oregon State (participant) Game Ticket, and Halftime Performance DVD. All participant fees must be received 5 (business days) prior to the clinic date. If your payment is not received by this date, your child will be unable to participate. Upon arrival, you will receive your game tickets and your child's Cheer Fest t-shirt. This t-shirt MUST be worn during the performance.

Coaches Information

Up to two coaches per group will receive a complimentary t-shirt and access to the field during the halftime performance. Coaches may secure seating with the group by indicating the necessary number of additional tickets below.
