We invite you to join us for the LHHG Sustainability Block Party Oct 22-23, 2011 at 11 Noon- 7:00p.m. beginning at 45 Bennett Street Design District, Atlanta, (Buckhead), GA 30337  

By signing this form:

1)  I  agree to participate in Living Healthy Happy and Green Sustainability Block Party and abide by the rules, regulations and procedures.

2)  I will hold harmless the organization’s staff, volunteers, and sponsors.

3)  I commit to the above stated requirements and understand my rights as a participant and give authorization to the Living Healthy Happy and Green Corp to record any and all parts of my image, voice or likeness. I waive the rights to any footage, photos, audio of my voice; image or likeness captured for any portions of this event and will arrive at the specified time schedule to participate.  

4) All individuals and media will provide LHHG with any and all copy (s) of any and all footage, photos, audio captured during the Block Party and written consent for commercial use of any and all recording (s) must be in writing with original signatures by company representatives.  All footage must be copied in a format requested by Vanessa Turner.
