Allergies/Dietary Concerns *

Please note and relevant allergies or medical conditions along with any Dietary needs you have. We will do our best to accommodate all forms of dietary needs at our events, but if you have strict dietary guidelines please either email the staff directly at logistics@mnalliance.com or plan to provide at least some of your own food. Our kitchen facilities will be available for food production but are not an allergen free environment.

Build and/or Goblin Stamp Expenditures *

Do not request a copy of your card by this form, please email logistics@mnalliance.com for that. You may spend up to 50 Goblin Stamps per game day on production items (ie: 100 GS may be spent for the average weekend).

Spells Memorized

Please note spells you wish to memorize for each level for the first day of the event. This list will make up your Battle Board for the Event. If you have High Magic from your Formal Magic, please include your High Magic selection here as well. If you are NPCing, you do not need to include anything.

Magic Items *

Please include item number, rep number, restriction (Restricted or LCO), description, and effects as listed on the magic item tag (Example: SMN-MI00873. SMN0923P. LCO. Green beaded necklace with black stone pendant. 2/d Disarm (5/16/2016).) Please include times/ever magic item effects as well. These will be used to check your items in at logistics. If you do not have any magic items, please write "None".

All Charge Tags Needed for Magic Items

Please provide a compiled list of all of the magic item effect charges you have. (IE: Disarm x 3, Cure Serious Wounds x 2, Life x 1, Cloak Binding x 1). If you do not need any tags, please write "None".

Production *

Please include what production items you will be creating, if any, as well as the total cost and any batch information if applicable. If you are not using any production skill, please write "None".
