Recently a court ruled that the New York City Department of Education must release all Teacher Data Reports in response to a Freedom of Information Request by several media organizations.

WNYC Radio and The New York Times intend to provide the reports to the public -- and we want to include each teacher's response to his or her teacher data report as well.

If you are a teacher who has received a Teacher Data Report, filling out this form now ...

• Will help WNYC Radio and The New York Times publish your response at the same time as your rating.

• Will help journalists at The New York Times and WNYC Radio ensure that any response to your rating comes from you, and not someone else.

Your single response here wil be available to both The New York Times and WNYC Radio, and may be published by either organization when the NYC Department of Education releases the Teacher Data Reports.

For verification, please go to the "By Gender" section of your Teacher Data Report and look for the line marked "Male." Enter the first number on that line, under "Number of Students." (If you have two Teacher Data Reports, use the same TDR you used for the previous question.)
