The blog post discusses ways you might use Google Forms and Google Spreadsheets in addition to Google Docs. I created this little form to highlight some aspects of that -- most notably that you can easily email the form -- but there's much more detail within the blog post. This is an excerpt from a slightly larger pre-assessment. I created this to familiarize myself with grid-style questions in Google Forms, but do welcome your input in this admittedly non-scientific survey.
