Reservation Use Request Form
Attention:
1) Use this request form for the following reservation use time schedule only:
Mon-Thurs, Sat: 10am-10pm
Fri: 10am-8pm
Sun: 4pm-10pm
2) Do NOT use this request form for the other time schedule.
3) Only small group leaders can make request.
4) Make your request at least 5 working days before the actual event date.
5) Request up to 2 weeks in advance only.
6) Each request is limited to a 2-hour slot only.
7) Confirmed request is NOT transferable.
8) Request does not guarantee availability. Church office will confirm availability via email.
