This form must be submitted in order to be completely registered for Cheer ALONG with a paid registration form.

I understand that attendance will be taken each week at practice. My coach will take attendance and turn in the information to the cheer board each week.

Attendance is required at practice. I understand that it is not fair to the coach or squad for a Cheerleader to miss practice and expect to cheer at our next game.

Tennis shoes must be worn to all practices and games. Acceptable shoes are tennis shoes with laces and tennis shoes with Velcro straps. No other shoes may be worn. I understand that my Cheerleader will have to sit on the sidelines at any practice or game when this rule is violated.

NO jewelry of any kind may be worn to any practice or game. This includes earrings, necklaces or bracelets.

Cheerleaders will come to practice dressed in appropriate clothing. Shorts and t-shirts are acceptable or sweats when the weather cools. NO dresses or skirts are allowed.

Cheerleaders will wait with the coach until a parent/guardian arrives to pick up.

I will encourage good sportsmanship by demonstrating positive support and respect for all Participants, Coaches, Officials (and their calls during games), Board Members, Staff and Opposing Teams at all times. I will, along with any guest of mine, refrain from any negative, vulgar, inappropriate or crude language/behavior while participating in, or watching any league sponsored event.

I will acquaint myself with the rules as distributed by the cheerleading program and agree to abide by and support these rules. I will direct all my comments and criticisms to board members and conduct myself in a calm, civilized manner when doing so.

I understand that the Fishers-HSE Youth Cheerleading Program will not tolerate abuse of this code of conduct. I understand that if this code is violated, the league shall have the authority to impose penalties that may include elimination of my participation in league activities even to the extent of not allowing me to be a spectator. I or my child may also be banned from league participation for up to 3 years depending upon the severity of the infraction.

Uniform Request Form

This form must be submitted to be completely registered for Cheer. NO EXCEPTIONS.

UNIFORMS: *

If my child needs a uniform, I will attend one of the 2 uniform fittings scheduled for this season. I understand that after the last uniform fitting on May 31, 2013, I will have to pay a late fee of $25 to have a uniform ordered. After June 3, 2013 no more uniform orders will be accepted. You will be removed from our rosters.

My child's uniform still fits and she will be wearing her black 2012 uniform for the 2013 Season.

My child needs a new uniform for the 2013 season.

I will need to purchase either a TOP or SKIRT for the Fall 2013 season.

WE WILL NOT CARRY ANY EXTRA INVENTORY FOR THE 2013 SEASON.

This special request form must be submitted in order to be completely registered for Cheer.

Your daughter may request to be placed with one friend this season. That friend in return must ask for your daughter. Please coordinate this ahead of time with the friend. Additional requests for friends to be placed on the same squad must provide a coach. We will provide all the supplies need to run a squad. We just need a mom or moms to step and help with the coaching. This new policy will help eliminate the vast differences in squad sizes that we have had in the last season.
