Thank you for taking this survey. This survey is to see how parents and students feel about their learning experience during the third grade year with Mr. Fisher.
This survey will help to make Mr. Fisher a better teacher.  Your feedback will help improve his instruction, his discipline, and will enhance the overall quality and effective practices found in his classroom.
Your opinion is very important. This survey was recreated from an old generic survey, in general it has about 15 - 20 questions. I chose to leave a couple questions about the school in general, and a few of the questions allow you to say anything that you want to about the teacher's effectiveness.  

Participating in this survey is completely voluntary, but will be much appreciated. The survey responses will be confidential. That means that your answers will not be viewed by anyone directly, besides Mr. Fisher himself.
