By filling out this form, I agree to use the equipment in direct accordance to the policies and procedures as dictated by the Occidental College Media Arts and Culture faculty and the Student Production Handbook.  Any equipment abuse or neglect will result in direct financial responsibility for any and all replacement or repair costs, and may well result in disciplinary action that could lead to the severance of some or all of the equipment privileges.  Note:  The number in parentheses next to the equipment indicates how many of that item the Cage has, not how many you will be receiving with your order.
