Cincinnati Regional Team Confirmation Form
Please use this form to secure your place in the Cincinnati Regional Tournament.  Collected information is stored on a secured (HTTPS) server.

Each team must submit their Confirmation Form WITHIN 30 days of being ASSIGNED to the sanctioned tournament in which it is participating.  Failure to submit your Confirmation Form within 30 days may result in the removal of your team from this tournament.

Programs with multiple teams at the Cincinnati Regional must fill out and submit a separate form for each team.  Per AMTA Rule 2.6, penalties will be assessed to teams that withdraw within 30 days of the start of the tournament.  The penalties escalate the closer the drop is to the start of the tournament.  

Many future communications regarding the tournament will likely be distributed only via email!
