27 | Youth on Staff Assault - A youth makes physical contact with a staff intentionally and knowingly. | |||||||||
28 | Youth on Youth Assault - A youth makes physical contact with another youth intentionally and knowing. | |||||||||
29 | Serious Incident - An incident that meets criteria for serious incident reporting: escape, use of force, OC spray, hospitalization, sexual assault, emergency situations, deadly force, man-made or natural disasters, death, hostage situations and IRC hotline calls. | |||||||||
30 | Campus Disruptions-Incidents that would stop normal operations that demand immediate attention but did not result any major injuries.( ex: youth running around campus, no use of force used no injuries, youth climbing on top of buildings resulting in the delay of movement until normal order is restored). | |||||||||
31 | Referral to security-Referrals to Security – Youth’s behavior that warrants a referral to security. Agency policy outlines the process for which a youth is admitted, extended or released from security. Note that all referrals to security are NOT admitted. | |||||||||
32 | Self-Referrals to Security – Any youth in a high restriction facility may request a self-referral to security for protection of harm, mental health issues or other issues as described by the youth. | |||||||||

34 | Staff to Youth Ratio Not Compliant –The staffing ratio is required to 1:12. A facility ratio may be out of compliance due to call-ins, serious incidents, FMLA, alternative work assignments, suicidal supervision for youth placed on 1 youth to 1 staff ratio and vacancies. | |||||||||
35 | Escapes - Unauthorized departures from a TJJD facility. | |||||||||
36 | Arrests - A youth is taken into custody by law enforcement due to probable cause youth committed a new offense. | |||||||||
37 | Directives to Apprehend - An agency document requesting law enforcement to apprehend a youth typically due to absconding/escape from a halfway house or failure to report to parole officer. | |||||||||

39 | Incidents - Situation that require notifications to immediate supervisors for information purposes that may not necessarily warrant a hotline call or serious incident report being submitted: minor injuries that were accidental, youth misconduct , suicidal behaviors, infractions while in the community that are not criminal (public school rule violations, incidents at youth’s place of employment, not making curfew) |
