To get a Rating of 5, you need your copies of your coaching and mentoring sessions, if you have a colleague who is a master teacher, ask the copies of the PMCF she used when he/she observed you. A copy of FGDs minutes can also be used to get a rating of 5.

​

I have provided templates for FGD minutes about coaching and mentoring for Quarter 1-4, all you need to do is to encode your details and provide the names of your co-teacher who undergone the mentoring and coaching. Let your school head/master teacher sign it.

To get a rating of 5, you have to secure evidences of feedbacks through chat, certification, or observation like PMCF or STAR observation, of your superiors or stakeholders about your good practices that uphold the dignity of teaching profession.

*Chat/Feedbacks from your colleagues because of your contributions to finish a task
