Deepen impact of A2M programs, build on foundational A2M work, implement through partners

Programs are ongoing and continually re-open with additional funding, which has been the case with all of them (other than CDCLI which is too new).

Will reach out to partner network if eligible for programs that open up

DPP – role of providing free healthy food to participants, implement curriculum via online/mobile-based platform

CDCLI- provide clinic leaders with training and coaching to deliver higher quality dental care, first continuing education program of its kind

Medication Adherence- assess role of texting reminders in prescription adherence, behavioral health, orgs, depression screenings

PCMH- help FCCs get National Committee for Quality Assurance accreditation, huge reorientation of how clinics provide care, goal of coordinated/accessible, high quality care
