ข้อบ่งชี้ในการใช้ double Lumen UVC (Fr. 4)

ทารกควรมีน้ำหนัก > 1 kg (ยกเว้นได้ตามความเห็นของแพทย์)

(ไม่ควรใช้สำหรับ isovolumetric exchange transfusion เพราะอาจเกิด recirculation จากเลือดที่ infuse เข้าไปถูกดูดกลับออกมา)

1. Critical congenital heart disease

2. Persistent pulmonary hypertension

3. Hypoxic ischemic encephalopathy requiring therapeutic hypothermia

4. Significant hemorrhage or placental abruption

5. Prolonged resuscitation

6. Septic shock

7. พิจารณาในทารกที่หนัก < 1 kg หรือ GA < 28 สัปดาห์และไม่สามารถใส่ UAC ได้

การดูแล double lumen UVC

  1. Infusion rate อย่างน้อย 0.5 mL/hr สำหรับทั้งสอง lumen
  2. หากมี lumen ที่ไม่ได้ใช้ต้อง infuse NSS/2 ด้วยอัตรา 0.5 mL/hr อย่างต่อเนื่อง ยกเว้นใช้ positive pressure needleless connector ปิดปลายสายไว้สามารถใช้ heparin lock ได้ (ไม่ควรใช้ connector ชนิดนี้พร้อมกันทั้งสอง lumen)

ข้อดีและข้อเสีย

  1. ลดความจำเป็นในการเปิด peripheral IV ในช่วงสัปดาห์แรกของชีวิต
  2. อัตราการติดเชื้อ (sepsis หรือ catheter related blood stream infection) ไม่ต่างกันกับ single lumen
  3. อัตราการเกิด thrombosis, catheter malposition, NEC และอัตราการตายไม่ต่างกันกับ single lumen
  4. Multiple lumen catheters associated with increased risk of catheter malfunctions�
