DHHS CONTRACT

Training Requirements

DHHS require all direct care, TFC parents, support staff and volunteers receive training in various topics (25) prior to working with youth.

Training must be conducted annually as well (8 topics). Some topics are repeats of preservice training topics.

UYV must have a process to demonstrate that skills were learned from the training and for individuals to provide feedback about the training.

Training about the DHHS contract requirements is one such training requirement.

DHHS CONTRACT�Background Checks

    • All individuals living in the home age 18 and older must bave a background check. This includes completing paperwork and getting a live scan of fingerprints.
    • Minors ages 12-17 living in the home must have a name based background check (no fingerprints).
    • Adult background checks are renewed annually by the Office of Licensing. Scans of fingerprints do not need to be redone unless your license expires.
    • Individuals without a cleared background check cannot have unsupervised contract with a client.

  • Youth identifying information- confidential & must be safeguarded
  • Youth information shall not be released to anyone not providing services to the youth except as explained below:
    • Parents must give permission to use youth pictures in any public forum including social network sites (if have rights & can be located)
    • Caseworker may provide written permission to use youth pictures in a public forum if parents’ whereabouts are unknown, if parental contact cannot be made, or if parents do not retain parental rights
    • Youth must also give permission if at least age 8.
    • only general information about youth can be used. Youth identity must be protected. Even with permission to use pictures, faces must be covered.
    • No specific information, treatment issues, history, etc can be used that will identify the youth.
    • The caseworker must provide approval for a youth to use the internet, email, & social networking sites.

DHHS CONTRACT�Youth Safety

    • Youth have a right to be free from abuse and neglect while in your home and the UYV program.
    • Youth have the right to be free from retaliation for reporting any abuse to themselves or others.
    • UYV has internal methods for you and/or youth to privately report abuse. Your consultant can provide this information to you.
    • Physical restraints are prohibited as is any type of punishment that would harm a child physically, emotionally, psychologically–no coercion or humiliation, nothing that inflicts pain or fright, no deprivation of sleep or meals.. Please referral to the licensing rule R501-12-12 for additional information.

DHHS CONTRACT�Payment

    • The daily rate is for the cost of the youth care (room & board, meals, clothing, personal needs, family phone calls, routine transportation, school supplies, etc),
    • Youth are to eat nutritious meals with the family. Personal money can be spent on food for such things as snacks at the movie when with friends.
    • Payment starts the first day the youth is in your home.
    • The youth should qualify for free school meals. Talk to the caseworker and/or school.
    • The youth should have Medicaid to pay for medical & dental expenses

Home Capacity

  • Youth with different risk levels (low, moderate, high) should not be placed in the same home.
  • Youth more than four years apart in age should not be placed in the same home unless they are part of a sibling group.
  • DHHS youth from different divisions may not be placed together at proctor level of care without written approval from regional and program directors.
  • Youth of different genders cannot be placed together (unless a sibling group or parent & child).
  • Contract allows only one client in home (for 1st year- newly licensed family); two or three youth are allowed thereafter depending on home size and youth referred.

DHHS CONTRACT�Care & Supervision

    • TFC parents must be available to provide daily care & supervision of each youth.
    • Care & supervision of youth includes the same as provided to one’s own children such as general guidance and behavior management to assist the youth in developing skills appropriate for age & development.
    • TFC parents will treat the youth as part of the family & incorporating the youth into family activities & also be given the same responsibilities and expectations given to other family members.
    • TFC parents will ensure youth receive prompt medical care when injured or sick.
    • TFC parents will allow youth to participate in school and cultural activities.
    • TFC parents must be available 24 hours a day.
    • TFC parents must be immediately available to respond to an emergency; and on-site during nighttime sleeping hours

DHHS CONTRACT �Family Visits

    • Family visits (including sibling visits) must be facilitated by Contractor.
    • Family visits cannot be withheld as a consequence for youth behavior
    • The CFT will determine the visitation plan. DHHS encourages 3 visits per week based on the family’s schedule. Visits can be in person, phone calls, video chat, etc. as determined to be appropriate based on age and reunification plan.
    • Written approval for visits must be obtained from the caseworker prior to the visits occurring. The consultant should obtain the approval and will need the date/ time of visit(s), and transportation arrangements
    • The TFC parent and the youth parent should meet after the visit and discuss the visit to help facilitate positive transitions.

DHHS CONTRACT�Youth Absences

    • UYV may bill for a youth absence from the TFC home if the youth is absent for a full 24 and there is a plan for youth to return.
    • Contractor may hold a placement if the absence is part of a planned transition. Time spent in detention or AWOL does not count as a planned transition.
    • Absences beyond 8 days must have prior written approval from caseworker
    • Reason(s) for absence(s) must be documented
    • Contact (by phone or other means) must be maintained with youth and responsible party during the absence
    • Contact will be documented in youth’s file
    • Visit contract form MUST be filled out for any visits outside of TFC home regardless of the length of time.

DHHS CONTRACT�Transportation

    • When transportation of youth is more than 60 miles round-trip for family visits, court hearings/reviews or health services, mileage reimbursement shall be received by the DHHS referring agency.
    • Mileage reimbursement for other purposes requires prior written approval from caseworker and must still meet the over 60-mile criteria
    • Mileage reimbursement is allowed for a single trip regardless of number of youth transported
    • Mileage reimbursement forms must be submitted monthly to your consultant.

DHHS CONTRACT�Youth Discharge

    • Discharge planning starts at the time of admission to the program.
    • The discharge plan is determined by the CFT, the youth’s age and permanency plan.
    • When a youth is discharged from UYV, a personal needs inventory is completed and all the youth’s items are packed in suitcases or boxes (not trash bags) to be taken with the youth.
    • If the youth is AWOL at time of discharge, a copy of the inventory list and packed items need to be returned to UYV or the caseworker within 5 business days of the discharge date.
    • Upon a youth’s discharge, all outstanding paperwork for youth must be turned in to your consultant as directed. Do not keep discharged youth’s identifying information.
    • The contractor/TFC parent is responsible to replace any of youth’s belongings that are not properly accounted for or secured.
