Problem Statement

There is concern about automation bias with the use of large language models in medical diagnosis.

Solution

At a clinicopathologic conference (CPC), a patient’s case is presented. The discussant is tasked to present a diagnostic approach and the most likely diagnosis given the patient’s data.

At this ChatGPT-assisted CPC, the discussants were a professor emeritus and a third year internal medicine (IM) resident, assisted by ChatGPT.

CPC Format

    • The moderator discussed prompt engineering and CPC format.
    • Discussants took turns in listing their differential diagnoses as the case unfolds. The IM resident lists his differentials before he can ask ChatGPT.
    • The pathologist discussed the autopsy findings and the final pathologic diagnosis.

Outcome

Despite the junior physician's limited experience, the use of ChatGPT allowed him to closely approximate the diagnostic reasoning of the senior physician.
