Inclusion Officer receives pupil details

Inclusion Officer holds a specialist admissions person centred meeting including all specialist setting leaders meeting with Inclusion team, EPs and Partnerships e.g. outreach team

Specialist admission meetings allocates places to pupils across all specialist settings

Letter sent by Inclusion Officer to parents suggesting they visit Crownbridge School Letter informs them they should then advise LEA of their decision.

Outcome of visit discussed at next Specialist Admissions meeting between Inclusion Officer and School. Places are confirmed.

Inclusion Officer sends letter to parents advising them of decision.

Inclusion Officer confirms outcome with Crownbridge School in writing.

Transition should be a pro-active, planned process that is able to meet the needs of individual pupils.

Pupils and Parents will be as fully involved as the practically can be.

The Local authority decides and prioritise placements. There are a range of processes that take place before a placement is considered by the Local Authority, including Placement Panel.

The Specialist Placement Panel Consists of:

The decision is based upon the evidence provided to the Panel using both the threshold and process criteria.

The Panel outcome is recorded, and once finalised, written confirmation of the Panel’s decision is sent to the school or pre-school setting and to the parents / carers

Transitions out – Careers and work-related experiences run throughout the curriculum at Crownbridge and pupils have the opportunity in Year 11 and Year 13, alongside Pathway courses. Future destinations will be discussed during PCP meetings and the annual review process. This generally happens Post 16, if it is felt that Crownbridge is no longer the correct placement for the pupil this can be at any stage of the pupils Crownbridge Career. Post 16 transitions involve Coleg Gwent, Social Services Transition lead and Careers Wales. The Transition lead will meet with you to start planning in the Summer term of Year 13.

  1. Parents/carers invited to attend the initial transition meeting at Crownbridge
  2. Future destinations discussed in annual review process
  3. Names of Crownbridge transition lead shared with parents and potential future settings, future provision transition lead names and contact details with parents and Crownbridge class teacher for initial visits to be arranged.
  4. Transition lead and class teacher to offer support with application forms, if needed.
  5. Where it is decided that a young person does not have capacity, then the multi agency team working with the young person will hold a best interest meeting.
  6. Once placement has been agreed, transition action plan arranged with parents and future setting – if required wider professionals will share information with future setting.
  7. Parents signposted to webpage or literature of future setting
  8. Transition lead to share key documentation with the future setting including IDP, One Page Profile, Learning Profile and progress towards Individual Outcomes.
  9. Learner visits to be planned on an individual level
  10. Transition lead will check in with parents following transition visits
  11. Transition lead to link with new placement during the Autumn Term check transition progress

Transitions in

  1. Names of Crownbridge transition lead and designated SLT member shared with parent/carers and current setting.
  2. Crownbridge SLT attend PCP of learner at current setting to gather information and liaise with other professionals, all information should be shared with Crownbridge staff and added to central electronic folder.
  3. Parent/carers sign posted to website where photographs of the school and social stories are available.
  4. Parent/carers receive welcome pack and supported to fill out all relevant Crownbridge forms.
  5. Learner visits to be planned on an individual level – support from any current setting required.
  6. Learner invited to the Torfaen moving up day – planned on a individual level.
  7. Relevant documentation from current setting to be shared with Crownbridge.
  8. Crownbridge to prepare a one page profile in conjunction with any other information to plan appropriate provision for September.

Pupils will be at the centre of the transition planning process
