Transitions
A transition is an exciting time for our families. It is a sign that your child is growing older.
It is an opportunity to reflect on your child’s needs and the next steps in your child’s education. Your child’s transition is unique to your child’s needs and growth.
We recognize that this is an exciting time and attending this meeting is the first step for your child’s transition.
Transition Presentation Goals
Our goal with this presentation is to discuss transitions for our students.
We will discuss the following:
Transition Points: When They Occur
Preschool into Kindergarten. IEP meeting in the spring
SDC Kindergarten - 2nd Grade. IEP meeting in the spring
SDC 3rd Grade - 5th Grade. IEP meeting in the spring
SDC 6th Grade - 8th Grade. IEP meeting in the spring
Speech preschool - Kindergarten. IEP meeting in the spring
RSP/Speech 5th grade at a K-5 school. IEP meeting in the spring
RSP/SDC/Speech 8th grade at a middle school or k-8 school. IEP meeting in the spring

The focus of the IEP meeting in the spring will be to review your child’s progress with goals as well as present levels and to discuss the transition to the next educational program.
The discussion will include:

The focus of the IEP meeting in the spring will be to review your child’s progress with goals as well as present levels and to discuss the transition to the next educational program. The IEP team will coordinate with the high school district for a representative from the high school district to attend.
