Student Athlete Clearance Requirements

Students must have a 2.0 GPA (recent quarter/semester grades)

Parents register their students online through Home Campus Registration (www.HomeCampus.com)

Complete a baseline concussion screening (dates TBD)

(Sports that require a Baseline are Football, Flag Football, Cheer, Volleyball and Water Polo)

Physical form needs to be completed by physician FORM

Step 1: Go to HomeCampus.com and make an account

Step 2: Click ‘Start Clearance’

Step 3: Start filling in all student and medical information

Step 4: Upload files - Click the Blue + to upload documents

Baseline Concussion is good for TWO years from test date. You will upload a photo of your email confirmation of completion that lists your name, date and testing ID #

Sports Physicals MUST be signed, stamped, and dated by the doctor. They are good for ONE year from the when the doctor signs. The doctor must check the box that says ‘Cleared to participate with no restrictions’

Immunization Records are NOT a Sports Physical

Physicals and Baselines are transferable between sports

Keep this file saved as it will have to be re- uploaded the following year.

How to Register Closing

Step 5: Print the last page of your online registration that says ‘Confirmation’

This is your contract and must be signed by both student & parent and turned into Activities on your sport’s Contract Submission Date
If you can not make your sport’s designated day, then you must wait to turn in your contract till the makeup date

Athletic Registration Dates

These are the designated dates to turn in your athletic contract to Amador Activities. Please only show up on your sport’s designated submission date. If you can not make it to your date, please come on the make-up date. Registration will open online prior to contract collection

Teams - every team must have:

Team Manager - Responsible for coordination between coaches & parents regarding schedules, pictures, fundraising etc. Will work with AV Athletic Boosters Director of Team Managers. Will also work with AV Athletic Boosters VP of Communications to provide up-to-date information for the Team Area on AV Athletic Boosters website.
AV Athletic Boosters Representative - Will be the contact for AV Athletic Boosters for the Purple and Gold Ball - Team Baskets and Team Donations. Will work with the Team Manager.
Team Treasurer - Responsible for creating and managing team budgets with the coaches. Will work with the Team Manager.
