Letter to families

    • Send home the introductory letter explaining the process, rationale, and ultimate goal. The letter should explain the collaboration between student, home, and school.

Develop the plan

    • Use the data collected to fill out the yearly plan. An overview detailing the skills needed to learn in order to engage in the identified activity.
    • Develop a measurable goal and objectives with student/family participation and/or approval.

Develop the plan

    • Research viable locations, the cost, distance, contact information, method of transportation, and equipment needed.
    • Discuss frequency and time that the student will engage in the activity.

Resources to support the family

    • Develop a community resource for the family such as (but not limited to): organizations in the area, transportation options, financial support for engagement in activities, places to get the necessary equipment needed.

    • Picture schedule board
    • Picture communication symbols
    • Visual reminders

Monitor the transition plan and growth

    • Monitor practice of the skills (goal/objectives)
    • Check-in with the family and guide the student/family contacting the facility and the questions to ask
    • Have the student maintain a log of their engagement
    • Constantly collaborate with the educational team

Review and revise the plan

    • Meet with the student, family, and educational team to evaluate the document and any new data collected
    • Revise the document to reflect the student’s present performance and growth toward engagement with maximum independence in the community.
