What’s teletherapy like with Daybreak?

You’ll love your therapistOnce you've been referred to the Daybreak program, you will schedule a 50-minute intake appointment via Zoom with a Care Coordinator. This call is intended to help find you the perfect match in a therapist. During this conversation, they will ask you some general questions about yourself as well as more specific questions around your hobbies, interests, culture, goals, and more.

We work really hard to match you with a therapist that meets your needs. Someone that you will be comfortable with, like, and can relate to.

You can get started ASAPOnce you are matched with a therapist—which usually happens in just a few days to two weeks—you will go ahead and set up your first session.

Meet where you’re most comfortableBecause Daybreak is a teletherapy program, you have the flexibility to meet with your therapist wherever it’s most comfortable for you. All you need is your phone, tablet, or computer and a private, secure place where you can talk openly and freely with your therapist.

It’s all about youYour first session is all about getting to know one another and building a trusting relationship with your therapist. In future sessions, you’ll set personal goals, talk about challenges you have, and learn ways to build the skills you need to thrive in the future. Typically students complete Daybreak’s teletherapy program in 12 weeks.

We’re here for youDaybreak is committed to providing you with the best experience and care possible. That means you can text with your therapist in between sessions if you feel like you need some extra support. It means our Care Coordinator will send you appointment reminders via text and other updates. It means that we are committed to helping you feel better and live happier.

After intake for referral�Once a student has been referred to the program, a clinician on the Intake Team will complete an intake assessment to identify if the student will benefit from the 12 week program. This assessment is the first step in beginning their journey and plays a critical role in the matching process.

From intake to clinician match

If the student is eligible for services, they will be moved into a matching process where the intake data (clinical focus, student hobbies, availability and preferences on clinician) is analyzed to ensure we match the student with the best clinical fit. Students are not just assigned to clinicians based on availability; Daybreak Health evaluates all aspects of the student’s needs to match them with the best possible clinician and ensure the student has the greatest opportunity for program success. If the student is not suitable for services, the intake specialist will touch base with the school referring staff and provide details around the decision and potential next steps.�

Care extensions and discharge from services

As the student’s 12-week program with Daybreak winds down, In preparation of discharging a student the clinical team assesses if a continuation of care is going to be required. This is done a few weeks in advance to give adequate time for referral arrangements without disrupting care. Once the student is ready to be discharged from the program, we close the loop by having the clinician provide a discharge summary form of the student’s care and any additional information that may assist the school counselor on how to best support the student moving forward. This information is provided via PDF and also can be exported into reports for district level viewing.

Client Success Team guidance�Throughout this journey, the Client Success Team supports and enhances the student experience, guiding the student towards their first session (e.g., sending email and text reminders), ensuring the collection of feedback on the therapeutic alliance and clinical impact, and facilitating the right parent and school check ins (see ‘Communication Touchpoints’ on the next page).
