Medically Tailored Meals

Nutrition Counseling: Any combination of educational strategies designed to motivate and facilitate voluntary adoption of food choices and other food and nutrition-related behaviors conducive to health and wellbeing

Services may consist of:

Medical nutritional therapy assessment or reassessment
Nutrition education or information (evidence-based)
Meal preparation education in an individual or group setting

Home Delivered Meals: receipt of prepared hot foods, meal kits, or restaurant meals

Services may:

Take into account a member’s household size
Be administered through a voucher or prepaid card
Dietary Guidelines for Americans to encourage healthy food selection.

Pantry Stocking: Groceries that may be picked up or delivered to the member’s home

Services may:

Take into account a member’s household size
Be administered through a voucher or prepaid card
Dietary Guidelines for Americans to encourage healthy food selection.

Medically Tailored Meals: Meals tailored to support specific individuals for which nutrition supports would improve health outcomes including preparation, provision, and delivery of the meal

Services must:

Preparation and provision must be consistent with a nutrition care plan, 3 meals a day for up to 6 months
Each meal must contain sufficient food to support approximately one-third of the individual’s daily nutritional need.
