What is the Moving Forward Coalition?
The Coalition is a group of individuals and organizations that have come together to drive specific, actionable improvements in nursing home quality, while building a sustainable network to continue this vital work.
In the first two years, we are developing, testing and promoting action plans to advance initiatives to improve accountability and financing, strengthen the workforce, and expand equitable, resident-driven care.
We are funded by The John A. Hartford Foundation.
Coalition Vision and Mission
�Our Vision is that every nursing home will be a community in which lives are nurtured, residents are empowered, and people want to work.
Our Mission is that we will make vital changes in policy and practice through the power of bringing diverse voices together now.
Goal 1 | Deliver comprehensive, person-centered, equitable care that ensures residents’ health, quality of life, and safety; promotes autonomy; and manages risks
Goal 2 | Ensure a well-prepared, empowered, and appropriately compensated workforce
Goal 3 | Increase the transparency and accountability of finances, operations, and ownership

Vision for Residents: Residents will have the opportunity and support to make choices that drive and shape their own care, experience and life.
Committee #1 | Test reliable and consistent processes and systems in which resident GPPs are described and documented on admission and as needed. |
Committee #6 | Identify and test a measure to assess care concordance with GPPs. |
Committee #7 | Design and test HIT infrastructure to collect and share residents’ GPPs. |
Committee #5 | Test a collaborative QA program to provide oversight and evaluation of systems to identify, document and implement residents’ GPPs. |
Workforce
Vision for Residents:
Residents will receive �care and support from a well-trained, caring and consistent team of providers who are appropriately compensated.
Initiatives We’re Exploring:
Developing a resource guide for state administrators, nursing home leaders, and others to facilitate their implementation of existing programs and initiatives that addresses wages.
Proposing a framework for states to integrate workforce metrics into state-based quality incentive payment programs.
Working with the Department of Labor and Health Resources and Services Administration (HRSA) to improve nursing home employer and employee access to existing federal workforce development, training, and loan forgiveness programs.
Finance
Vision for Residents:
Nursing home residents will live in communities with�greater privacy, dignity, and more consistent staffing.
Our finance committee is working with federal agencies and professional associations to discuss possible opportunities for collaboration.
Accountability
Vision for Residents:
Nursing home residents will know who is responsible for the care they receive, be able to make choices based on that knowledge, and trust government to monitor quality.
We’re trying to make sure that we can get the right, accurate, honest information in the hands of the right party at the time they need it and when they can make a decision about it. �

2023 Approaches and Outcomes
June 30, 2023 | Seven or more action plans will be developed |
September 30, 2023 | We will further develop, test, and advocate for at least 3-4 action plans |
October 31, 2023 | We will confirm co-funding/sponsorship relationship with at least one organization |
December 31, 2023 | We will begin to demonstrate how initiatives advance equity and consistent quality. |
MOMENTUM has a distribution of about 750 participants and organizations.
