MOR Summary Sheet

  • Public Water System Information:
    • Green Cells are pre-populated. Complete the following:
      • Month and Year
      • Contact Person, Email, Phone, and Comments
  • Turbidity Combined Filter Effluent (CFE):
    • Nothing to complete. All information generated from data sheet.
  • Microbial Treatment:
    • Nothing to complete. All information generated from data sheet.
  • Entry Point Residual (if applicable):
    • Section removed if not required.
    • Enter the number of samples taken.
    • Answer if the residual dropped below 0.2 mg/L for more than four (4) hours (yes/no).
  • Turbidity Individual Filter Effluent (IFE) (if applicable):
    • Section removed if not required.
    • Answer the 7 questions
    • If the answer to question 4, 5, 6, or 7 is Yes then please visit wqcdcompliance.com/forms and complete/submit the IFE report form.

  • General
    • Accepted Values: Numeric >= 0, NS (Not Sampled) and PO (Plant Offline). All other values converted to Blanks.
    • When plant is offline for the entire day place PO in all 6 time periods.
    • C (Continuous) is allowed for Disinfection time periods only. If used, the Lowest Daily Residual Sample Column must be completed.
  • Turbidity Combined Filter Effluent (CFE) Data Sheet
    • Enter value for each time period.
      • If multiple results in a single time period then enter the highest result for the time period.
    • The highest individual daily turbidity sample is automatically entered.

Disinfection or Log Inactivation Data Sheet

Type 1 - Microbial Treatment – Minimum Chlorine Residual

      • On same sheet as CFE.
      • Enter sample for each time period.
        • If multiple results in a single period then enter the lowest result for the time period.
      • Enter the lowest individual daily disinfection residual.
        • If entered, the lowest time period sample will be used.
      • If monitoring continuously then enter “C” in the time periods and enter the lowest individual daily residual.

Type 2 - Microbial Treatment – Log Inactivation

      • Enter samples for each time period for:
        • Peak Hourly Flow (gpm), Contact Tank Volume (gal), pH, Temperature (deg C), and Residual.
      • The lowest individual daily ratio it automatically entered, unless there is already a lower value entered.
    • For Peak Hourly Flow (gpm) - Labeled as Flow (gpm)
      • This value should be calculated based on the highest volume created in a one hour period divided by 60 minutes.
      • DO NOT USE THE VOLUME PRODUCED OVER A DAY DIVIDED BY 1440 - that is incorrect.
    • For Contact Tank Volume (gal) - Labeled as Volume (gal)
      • Please Enter the minimum VOLUME of the tank where disinfection is occurring - so if you have a clearwell that holds 100,000 gallons, but it was only 90% full during the period - enter 90,000 gallons.
      • DO NOT ENTER VOLUME PUMPED DURING THE DAY
    • For pH and Temperature
      • You may collect as infrequently as once weekly - however please use the higher pH and the lower temperature if these are not being measured every 4 hours.
    • For Chlorine residual (mg/L)
      • Please use the minimum chlorine residual read during the 4 hour period in question.
