Teacher Section 1:

Individual Evaluator Assessment

1. Access Student, then SSS

SSS is across the top menu.

• Open ETR Meeting from Active Events

Click VIEW to access

Teachers who need to complete the Section 1: Individual Evaluator’s Assessment for an ETR Meeting can access the form through the SSS Module in Focus.

• Select Section 1 from left-hand menu

• Complete Assessment Form

Teacher Section 1:

The form will load; complete all applicable fields. NOTE: Some sections may require you to click EDIT in order to edit the form

• Sign Evaluation Form

Teacher Section 1:

Individual Evaluator Assessment

At the bottom of the second page, enter a date and click on Click to Sign.

In the popup window, use your mouse, touchscreen, or touchpad to sign your name. Click Sign when complete.

• Save Form

When form is complete and signed, click Save on the right hand side of the screen.
