BỆNH KAWASAKI

Là bệnh viêm mạch máu trung bình

Thường xảy ra ở trẻ < 5 tuổi:

• Hồng ban ở da đa dạng
• Viêm kết mạc mắt 2 bên không xuất tiết:không ghèn, không mủ
• Viêm niêm mạc miệng, môi khô, lưỡi dâu
• Sưng phù mu bàn tay bàn chân
• Hạch cổ 1 bên >1,5cm

KAWASAKI

• When treated appropriately, KD is associated with a low mortality rate.
• 25% of untreated patients and 5% of treated patients developing coronary artery aneurysm
• Among infants < 6 months of age, the risk of a coronary artery aneurysm is 50%

KAWASAKI THỂ ĐIỂN HÌNH

Sốt ít nhất 5 ngày, không giải thích được nguyên nhân khác và kèm 4/5 tiêu chuẩn lâm sàng:

• Viêm kết mạc mắt 2 bên không xuất tiết
• Môi khô đỏ nứt, lưỡi dâu, hồng ban ở niêm mạc miệng, niêm mạc hầu
• Sưng phù mu lòng bàn tay, bàn chân ở gđ cấp, hoặc gđ bán cấp là tróc chân
• Hồng ban đa dạng ở thân mình
• Hạch cổ 1 bên, đường kính < 1,5cm

AHA 2017: Có thể chẩn đoán Kawasaki sớm nếu sốt 4 ngày nhưng đủ 4/5 tiêu chuẩn vẫn chẩn đoán

INCOMPLETE/ATYPICAL KAWASAKI

Children suspected of having KD who do not fulfill diagnostic criteria (less than four signs of mucocutaneous inflammation) may have incomplete or atypical KD

Incomplete KD is the preferred term since these patients do not appear to differ from those with classic KD in any way except that they lack a sufficient number of criteria to fulfill the epidemiologic case definition.

Positive echocardiaography

• Zscore of LAD or RCA >=2,5
• coronary artery aneurysm is observed
• >= 3 features: decreased LV function, mitral regurgitation pericardial effusion, Zscore in LAD to RCA 2 to 2,5

IVIG

• Giảm nhanh phản ứng viêm
• Rút ngắn thời gian sốt
• Giảm tỉ lệ nguy cơ phình mạch vành

Mặc dù cơ chế tác dụng chưa rõ ràng

NGUY CƠ CAO KHÁNG IVIG

Điều trị: IVIG + Glucocorticoid

(AHA 2017, ACR/VF 2021)

• Zscore LAD và RAD >= 2.5
• Trẻ < 6tháng tuổi

--> IVIG +nonglucocorticoid HOẶC immunomodulatory immunosuppressive agents

KAWASAKI + MACROPHAGE ACTIVATION SYNDROME (ACR/VF 2021)

• Biến chứng nặng, hiếm gặp
• Nghi ngờ hoặc đã chẩn đoán xác định hội chứng đại thực bào --> điều trị IVIG và một thuốc điều trị hội chứng hoạt hoá đại thực bào

• Hội chứng hoạt hoá đại thực bào ở bệnh nhân KD: sốt kéo dài, lách lớn, tăng ferritin máu, tăng tiểu cầu
• Nên điều trị KD với một thuốc điều trị hội chứng hoạt hoá đại thực bào
• Anakinra and glucocorticoid

Tiêu chuẩn Ravelli

KD AND PERSISTENT FEVERS AFTER INITIAL TREATMENT WITH IVIG

A second course of IVIG in patients who have persistent fever for > 36 hours after the first dose is conditionally recommended, as it is the current standard of care

Glucocorticoids are a reasonable alternative (2mg/kg/day and tapering over 15 days or a single dose of 20-30mg/kg)

Current evidence does not clearly indicate the superiority of a second course of IVIG over glucocorticoids

PERSISTENT FEVER AFTER REPEATED TREATMENT WITH IVIG

Ungraded position statement:

For patients with acute KD and persistent fevers after repeated treatment with IVIG, either nonglucocorticoid immunosuppressive thearpy or glucocorticoids may be used

Combination therapy (glucocorticoid + nonglucocorticoid) can be considered in severe cases (rapidly expanding aneurysms or imminently lifethereatening disease)

DIAGNOSTIC IMAGING

• For children with suspected incomplete KD and fever, obtaining an echocardiogram with coronary artery measurements without delay is strongly recommended over not obtaining en echocardiogram
• For children with unexplained shock physiology, obtaining an echocardiogram with coronary artery measurements is strongly recommended
• For children with unexplained MAS, Obtaining an echocardiogram with coronary artery measurements is strongly recommended
