What is transition?

  • Transition describes a time of change when the young person with the support of their family make decisions as to which path to follow in the transfer from educational provision to adult life .

When does the Transition Process start?

Transition begins for your child at the age of 14+

Their Statement of Special Educational Needs is reviewed annually to allow for changes to be made , where necessary. A Transition Plan forms part of the first review following the young persons 14th Birthday.

Why start at age 14?

  • To establish a path that has been well planned for which will take into account all your child’s needs as well as well as the needs of the family as a unit
  • To ensure a smooth transition to future provision.
  • The Transition Plan will act as a tool which will steer the course of your child’s last years in school taking into account their aspirations for the future as well as yours.

What is a Transition Plan?

  • A Transition Planshould draw together information from a range of individuals within and beyond school in order to plan coherently for the young person’s transition to adult life’

The Role of the Education Transitions Co-Ordinator

  • To support the Young Person and their family throughout the transition planning process.
  • To advocate for the young person and their parents on issues which directly affect them.
  • To liaise with and support schools in the implementation of the transition plan.
  • To collaborate with various statutory and voluntary agencies in order to provide an effective and developing service.
  • To strive to overcome the obstacles which are prohibiting effective and smooth transition.
